Benefits to patients
Working in partnership under the East Riding of Yorkshire Health and Wellbeing Board, local organisations believe the jointly produced Better Care fund plan will lead to a wide range of benefits to the public.
- The new system will ensure people receive the right care and support at the right time, in the right place.
By working together staff from health and social care can easily identify patients most at risk of going into hospital. We can then put together an agreed combined package of treatment, care, support and advice that is not only provided as close to home as possible or in the home but also designed to help patients better understand and manage their condition. Patients are empowered to take ownership of their health, maintain it and remain independent for longer.
- If someone does end up in hospital staff can work with the local authority and those in the community to help them return home with the right support in place.
- More, improved services will be available in the local community. This will help to reduce the amount of preventable admissions to acute hospital, and long-term care as well as decrease the length of time patients stay in hospital.
